YOUTH WAIVER ACKNOWLEDGEMENT

By submitting Player Registration Form, participants parent/person having legal custody/guardianship give permission for the minor to participate in all Atlante San Diego FC programs. Submission of Electronic Player Registration Form will be used as Electronic Signature for Paper Youth Program Registration Form for all Atlante San Diego FC Programs.
